Hospitalist Program physicians are specialists who focus on the care of hospitalized patients. Hospitalists ensure continuity of care from admission through discharge, concentrating on each patient's unique, individual needs. Because hospitalists work entirely in the hospital setting, they can provide timely bedside visits and prompt follow-up on lab results and tests. This allows them to manage patients' care effectively and efficiently.
Patients who need skilled nursing care and are transferred to Northeast Rehabilitation in Nashua will also received care from by our Hospitalist team during their stay.

Foundation Medical Partners is a member of the New Hampshire Accountable Care Partners Accountable Care Organization. NH Accountable Care Partners ACO is a joint venture among four hospitals and ACO professionals. The ACO consists of more than 965 healthcare providers employed by Concord Hospital, Elliot Hospital, Southern New Hampshire Medical Center, or Wentworth Douglass Hospital who coordinate the health care of more than 30,000 Medicare Fee-for-Service beneficiaries in our combined service area. Click here to learn more.
